RESUMO

Objective:To study the effect of continuous subcutaneous insulin infusion(CSII)on the emotional disorder of children and adolescents with type 1 diabetes mellitus(T1DM)and their parents.Methods:A total of 72 children and adolescents with T1DM were divided into CSII group( n=40)and multiple daily injection(MDI)group( n=32).There were 58 healthy children and adolescents with their parents selected as control group.The emotional condition of children and adolescents in T1DM group and control group was evaluated by Depression Self-rating Scale for Children(DSRS)and the Screen for Child Anxiety Related Emotional Disorders(SCARED)respectively, and Symptom Checklist-90(SCL-90)was used for evaluating the mental health of all parents. Results:The average glycated hemoglobin A1c(HbA1c)of T1DM group was at the optimal level(7.406±1.294)%.The average HbA1c of CSII group was significantly lower than that of MDI group[(7.040±1.082)% vs(7.863±1.404)%, t=2.728, P=0.008].The depression rate of children and adolescents in T1DM group increased significantly than that of control group(31.9% vs 15.5%, χ2 =4.671, P=0.031).There were statistically significant differences among CSII group, MDI group and control group(20.0% vs 46.9% vs 15.5%, χ2 =11.591, P=0.003).The depression rate of children and adolescents in MDI group increased significantly than that of CSII group and control group(all P<0.05).CSII group showed similar results as compared with control group( P>0.05).Concerning the anxiety in children and adolescents, there was no significant difference between T1DM group and control group(19.4% vs 13.8%, χ2=0.730, P=0.393), and there were no significant differences among CSII group, MDI group and control group(15.0% vs 25.0% vs 13.8%, χ2=1.994, P=0.369).The emotional disorder prevalence of parents in T1DM group was remarkably higher than that of the control group(31.9% vs 5.2%, χ2=52.927, P<0.01).The factor scores of obsessive-compulsive symptoms, interpersonal sensitivity, depression, anxiety, hostility, paranoia and psychotic symptoms in T1DM group were higher than that of control group( P<0.05).There were statistically significant differences among CSII group, MDI group and control group(17.5% vs 50.0% vs 5.2%, χ2=26.126, P<0.01).The emotional disorder prevalence of parents in MDI group increased significantly than that of CSII group and control group(all P<0.05).But CSII group was same as that of control group( P>0.05). Conclusion:The children and adolescents with T1DM and their parents were high-risk population of emotional disorder.CSII can reduce not only the depression in the children and adolescents with T1DM, but also emotional disorder of their parents, thus CSII can improve the mental health in families suffering from T1DM.

Objective:To find out the prognostic influencing factors of patients undergoing continuous renal replacement therapy (CRRT) for refractory acute left heart failure.Methods:Through the medical system and hemodialysis system in Foshan First People's Hospital, all patients who received CRRT for refractory acute left ventricular heart failure from January 1, 2012 to January 1, 2019 were searched. All patients were divided into two groups by the final outcome: survival group and death group. Age, sex, initial mean arterial pressure (MAP), primary heart disease, use of vasoactive drugs, urine output before treatment, hemoglobin, serum creatinine, serum albumin, C-reactive protein(CRP), brain natriuretic peptide (BNP), cardiac ejection fraction (EF) and CRRT treatment time were analyzed to find out the prognostic influencing factors.Results:A total of 130 cases were collected, including 96 cases in the survival group and 34 cases in the death group, with a total mortality rate of 26.15%. Compared to that in the death group, there were higher proportion of males (71.88% vs 50.00%, χ2=5.366, P=0.021), significantly higher initial MAP ( t=4.677, P<0.001), much more urine output before treatment ( Z=3.904, P<0.001), significantly higher serum creatinine ( Z=2.866, P=0.004) , significantly lower hemoglobin ( Z=-2.587, P=0.011), significantly shorter time of CRRT ( Z=-3.447, P=0.001) in the survival group. Multivariate logistic regression analysis showed that female ( OR=2.950, 95% CI 1.102-7.898, P=0.031) and higher levels of hemoglobin ( OR=1.024, 95% CI 1.004-1.045, P=0.019) were the risk factors of death in patients undergoing CRRT for refractory acute left heart failure, while higher levels of mean arterial pressure before treatment ( OR=0.959, 95% CI 0.930-0.989, P=0.008) and urine volume before treatment ( OR=0.998, 95% CI 0.997-0.999, P=0.004) were the protective factors for patients' prognosis. Conclusion:The mortality of patients with refractory acute left heart failure undergoing CRRT therapy is still very high. Female and higher level of hemoglobin are the risk factors for death, while more urine volume before treatment and higher MAP before treatment are protective factors for survival.

Objective To explore the association between the GRIN3B gene and Tourette syndrome (TS) in children by screening mutations in the coding region of this gene.Methods Fifty-one children with TS and their parents in the Affiliated Hospital of Qingdao University from October 2015 to November 2016 were selected as an experimental group,41 cases of boys,and 10 cases of girls,aged 6-16 years[(9.78 ±3.64)years],while 60 people aged 22-45 years in the health examination center were selected in the control group,49 cases were male,1 1 cases were female,aged 22-45 years [(29.08 ± 2.89) years].DNA was extracted from 51 patients with TS,their parents and 60 controls.PCR was applied to amplify the encoding region of GRIN3B gene and Sanger sequencing was used to sequence,then GRIN3B sequencing results were compared with the NCBI gene encoding region sequence (NM_138690.2)to test whether these patients carried gene mutation and to verify the findings from their family.Results c.C460T gene variant of GRIN3B was found in 2 patients (p.P154S);c.T1187C (p.L396S) variant of GRIN3B gene was found in 10 patients and both of abnormal GRIN3B sites lead to changes in amino acid.The 2 peak sequencing maps were obtained by Sanger sequencing but nothing was found in their parents.Conclusion The mutation of GRIN3B gene may be related to the development of TS.

RESUMO

Objective To investigate the effects of hyperbaric oxygen on learning and memory ability in rats with a model of VPA autism and on the pyramidal cells in the CA1 region of the hippocampus.Methods An animal model of autism was established in the offspring of Wistar rats which had received a single intraperitoneal injection of sodium valproate at the 12.5th day of pregnancy.A total of 48 male VPA autism model rats were randomly divided into the hyperbaric oxygen group,a high pressure air group,a normal pressure high oxygen group and a normal pressure air group (each group with 12 rats).A normal control group was obtained by injecting physiological saline.The autism model rats of the hyperbaric oxygen group were treated with high pressure oxygen in an animal experiment cabin which was cleaned with pure oxygen for 10 min,pressurized for 15 min,held at 2.0 atmospheres absolute (ATA) for 45 min and then had the pressure relieved over 15 min.For the high pressure air group the cabin was pressurized with air for 15 min,held at 2.0 ATA for 45 min and the pressure was relieved over 15 min.For the normal pressure,high oxygen group the cabin was cleaned for 10 min with pure oxygen,then pure oxygen was supplied for 1 hour with the cabin door open.The normal pressure air group rats were placed in the open cabin with no pressure or additional oxygen.The testing lasted 7 days for 1 hour per day.The normal control group rats were placed in ordinary cages.Learning and memory were evaluated using the Y electric maze test before and after the interventions.Any changes in the pyramidal cells in the CA1 region of the hippocampus were observed after hematoxylin and eosin (HE) staining.Results The average number of tries of the hyperbaric oxygen group after treatment was significantly less than before treatment and memory retention times were increased.The number was also less in the normobaric hyperoxia group and memory retention times again improved.The number of apoptotic cells was reduced and the number of normal form cells in the CA1 region increased after the hyperbaric oxygen intervention.Conclusions The learning and memory abilities of the autistic rats improved after the hyperbaric oxygen intervention.Pyramidal cells in the CA1 region proliferated and the number of apoptotic cell decreased.This may be the mechanism by which hyperbaric oxygen intervention treats autism.
